Declaration of Terms
I understand and agree that:
i) PTR, their employees or agents accept no responsibility whatsoever in respect of personal injury, loss or damage, however caused, arising while attending any PTR function.
ii) PTR reserves the right to reject any applicant whom it considers to be an unsuitable person for involvement in coaching. This right may also be exercised retrospectively if information is brought to PTR’s attention, which, if known at the time of the application, would have rendered the candidate unsuitable. For the avoidance of doubt, the falsification of any information requested in this application form and/or the existence of any police record or caution relating to matters considered to be incompatible with certification, constitute circumstances where PTR reserves the right to withdraw or suspend a candidate’s qualification.
iii) Beginning in 2014, PTR is stressing the importance of Continuing Education. We are requiring each certified member to complete a minimum of 15 Continuing Education hours over a 3 year period. New members will join the current Continuing Education cycle. These hours can be in the form of attendance at PTR workshop, symposia and conferences as well as USTA, ITF and USPTA educational events. The hours can also be accumulated through digital education and distance learning through the PTR Education Dashboard and other outlets. Your PTR Certification Workshop will count for Continuing Education hours, the number of hours will depend on the course Level. The PTR Intl. Tennis Symposium will count for 30 hours.* So, start earning your Continuing Education Hours today!
iv) Cancellations received 96 hours prior to the start of the workshop will receive a 100% transfer of workshop and/or testing fees to a future workshop date/location. Cancellations received less than 96 hours and more than 48 hours prior to the start of the workshop will receive a 50% credit transferred to a future workshop date/location. Cancellations received less than 48 hours prior to the start of the workshop or failure to notify PTR Headquarters of your cancellation prior to the start of the workshop will result in a forfeiture of all fees. Loss of workshop fees will take place 12 months after registering. PTR on Campus participants who fail to notify PTR Headquarters of cancellation prior to the start of the workshop will result in forfeiture of the $10 workshop fee. Once forfeited, PTR on Campus members must pay the full Workshop Registration fee.
If I withdraw from a course once it has started, I will forfeit the course fee.
v) PTR reserves the right to cancel a course if enrollment numbers are not sufficient.
vi) I understand that in undertaking a course or obtaining my PTR certification, this does not provide me with a guarantee of work from PTR or any provider.
